FORT CAVAZOS, Texas – Currently limited to Active Duty only, Carl R. Darnall Army Medical Center’s Ophthalmology Clinic will reopen refractive eye surgery to limited career military fields. For those who qualify, this is an excellent opportunity to enhance individual readiness and lethality.
Service Members interested in Refractive Eye Surgery must complete the CRDAMC PRK/ LASIK Application Form Warfighter Refractive Eye Surgery Program form and gain their command’s approval. Please log on to the CRDAMC website at darnall.tricare.mil/Health-Services/Vision/Warfighter-Refractive-Eye-Surgery-Program-Laser-Clinic to download the form. Once complete, Service Members can drop off their paperwork to the Ophthalmology clinic on the 2nd floor of the hospital, Clinic 1 Hallway.
Service Members requesting Refractive Eye Surgery must meet the below requirements:
- Active Duty for at least 6 months
- Wearing glasses for at least a year
- 21 years or older
- No planned deployments for 6 months
- No adverse personnel actions pending
- Command Approval
- 6 months left in the military after surgery
- Priority 1 until further notice (see commander’s memorandum)
