FORT HOOD (Now designated Fort Cavazos)– Carl R. Darnall Army Medical Center has announced dates and locations for its annual sports physicals clinics.

Russell Collier Clinic will host their clinics from July 16-19 and July 30 through August 2. Hours are 4:30 -7:30 p.m.

Family Medicine Residency Clinic will host sports physical clinics July 10 and 24 and August 7 and 21. Clinic hours are 4:30-8 p.m.

CRDAMC’s Pediatrics clinic is offering sports physicals from 8 a.m. to 3 p.m. July 21 and August 11.

Beneficiaries can make CYS health assessments, school sports physicals, and immunization appointments at any clinic.

Parents  are encouraged to bring a completed school or CYS  health assessment form to the appointment.
